Notice is hereby given that pursuant to Chapter 373, Florida Statutes, the following application for permit was received on December 2, 2014:
McDonalds USA, LLC, Melvin Mance, 10150 Highland Manor Drive, Suite 470, Tampa, FL 33610 has submitted an application # ERP-125-222340-1. The project is located in Union County, Section: 31, Township: 5 South, Range: 20 East, and includes 1.9413 acres . The Environmental Resource Permit application is for Construction of a new 3,042 sf restaurant with drive-thru, 29 surface parking spaces, associated infrastructure with an on-site wet stormwater detention pond and Florida Native, drought tolerant landscaping. Known as McDonalds Restaurant – Lake Butler.
Interested persons may comment upon the application or submit a written request for a staff report containing proposed agency action regarding the application by writing to the Suwannee River Water Management District, Attn: Resource Management, 9225 C.R. 49, Live Oak, Florida 32060. Such comments or requests must be received by 5:00 PM within 14 days from the date of publication.

The City of Lake Butler reserves the right to reject any or all bids. The lease shall become operational upon approval by the City Commission. The City reserves exclusive rights.
