INVITATION TO BID

SEALED BIDS will be received and accepted by the Town of Brooker, Florida, until 12:30 p.m. on Monday, July 13, 2015 for the following mowing and cleaning of all City property. The bids will be opened and read aloud on Tuesday, July 14, 2015 at 7:00 p.m. at the regular Town Council meeting.

All Bidders are encouraged to attend a pre-bid tour to be held on Saturday, June 13th at 9:00 a.m. at the Brooker City Hall/Community Center. 17435 Tetstone Avenue, Brooker, FL. No other tour will be provided.

All Bidders are encouraged to submit references.

Monthly bid amount should be different for Number I and Number 2, due to less mowing in Number 2.

Bid #1 is for the following:

  1. Months of August 2015 through October 2015 and the month of April. May, June, July 2016 (7 months):
  2. Mowing and maintaining twice a month the Fire Station, Marshal’s office, F.D. Training Facility property, Water department and remainder of community center property.
  3. Mow and weed eat once a week on Wednesday or Thursday the town property around City Hall/Community Center.
  4. Mow and maintain twice a month the baseball field and tennis court area,
  5. Mow and maintain twice a month the areas within 12 (twelve) feet of the paved roads on all State, County, and Town Streets within the limits of the Town, unless not possible due to ditches or fencing or private planting, in these cases mow to fence line. All debris should be blown towards the highway.
  6. On Highway 18/Charlotte Avenue and Tetstone Avenue mow up to railroad tracks from old Marshal’s office to the Flashing stop sign twice a month.
  7. Weed-eating particularly the areas around the Fire Station, F.D. Training Facility property, Water Department, and Marshal’s Office and those other areas mentioned above that have sign posts, light poles, decorative trees, and all fire hydrants/stand pipes at least once a month.
  8. If bidder intends to spray any chemicals, then Commercial license are needed. The Town will provide the Round-Up and can be applied twice a year.
  9. Once a month remove garbage from cans at tennis court and baseball field.

Bid #2 same as above except once a month:

  1. Bid for the month of November 2015 through March 2016 (5 months) will be the same as specified above in sections (a) through (h) with the frequency listed as once a week and twice a month being changed to once a month starting from the date of the last activity in October 2015.

Bid will be for a two year period. The Brooker Town Council reserves the right to cancel with or without cause after one year. Payment will be rendered at the end of the month.

***Required with Bid – A copy of current Insurance Declaration page showing $250,000 or more Liability coverage and Worker’s Compensation coverage certificate or exemption certificate. BIDS NOT CONTAINING THESE REQUIRED DECLARATION PAGES WILL BE REJECTED.

The identity of the bidding entity must match the certificate of insurance coverage.

***AII Equipment including mower is to be furnished by the Contractor.

***Grass is not to be cut too short as to damage the root systems.

***No mowing is allowed on Sunday.

***There may be times when extra storm debris pickup will be necessary. A price will be determined on each occurrence

***Awarded bidder will be required to add the Town of Brooker as additional insured after receiving the bid.

The Town reserves the right to check references and to accept or reject any and all bids.

If Bidder has previously worked under contract with the Town of Brooker the award of the bid shall be contingent on satisfactory performance and review on contract previously awarded.

APRON REHABILITATION AT THE KEYSTONE AIRPARK

A non-mandatory pre-bid meeting will be held at 10:00 AM on Wednesday, June 3, 2015 in the Conference Room at the Keystone Airpark Terminal Building.

Bidders are invited to submit Proposals for this work on the Proposal Forms provided. Other proposal forms will not be accepted. The work shall include approximately 600 SF of concrete slab spall repairs, approximately 1,200 LF of routing and sealing of cracks in concrete slabs, removal and replacement of approximately 21,000 LF of concrete joint sealing materials, milling and overlay (2”) of approximately 5,000 SY of asphalt pavement, concrete curbing, pavement markings, aircraft tie downs, and related work.

The complete examination and understanding of the Contract Documents consisting of the Plans and Specifications, and all addenda or other revisions, and Site of the proposed work is necessary to properly submit a Proposal. There is a $50.00 charge for each set (half-size) of Contract Documents. Return of the Contract Documents is not required and the amount paid for the Contract Documents is non-refundable. A Bid Bond in the form as bound in the Contract Documents or Certified Check in the amount of not less than five percent (5%) of the total amount bid must accompany each Bid.

The public hearing may be continued to one or more future dates.  Any interested party shall be advised that the date, time and place of any continuation of the public hearing shall be announced during the public hearing and that no further notice regarding the matter will be published, unless said continuation exceeds six calendar weeks from the date of the above referenced public hearing.

All persons are advised that if they decide to appeal any decision made at the above referenced public hearing, they will need a record of the proceedings, and that, for such purpose, they may need to ensure that a verbatim record of the proceedings is made, which record includes the testimony and evidence upon which the appeal is to be based.

Notice is hereby given that pursuant to Chapter 373, Florida Statutes, the following application for permit was received on April 30, 2015:

David Shellhouse, of Florida Gas Transmission Company, 2405 Lucien Way, Suite 200, Maitland, FL 32751. Has submitted an Environmental Resource Permit (ERP) application number ERP-007-207592-4.  The 2.78-acre project is located in Bradford County, Section: 32, Township: 6 South, Range: 20 East.  The ERP application is to relocate and expand the existing permitted pond to account for the proposed expansion of the facility.  The total impervious area for the project is 0.77 acres.

Interested persons may comment upon the application or submit a written request for a staff report containing proposed agency action regarding the application by writing to the Suwannee River Water Management District, Attn: Resource Management, 9225 C.R. 49, Live Oak, Florida  32060.  Such comments or requests must be received by 5:00 PM within 14 days from the date of publication.

No further public notice will be provided regarding this application.  A copy of the staff report must be requested in order to remain advised of further proceedings.  Substantially affected persons are entitled to request an administrative hearing, pursuant to Title 28, Florida Administrative Code, regarding the proposed agency action by submitting a written request after reviewing the staff report.
