BY THE PLANNING AND ZONING BOARD OF THE CITY OF STARKE, FLORIDA, SERVING AS THE BOARD OF ADJUSTMENT OF THE CITY OF STARKE, FLORIDA, NOTICE IS HEREBY GIVEN that, pursuant to the City of Starke Land Development Code, hereinafter referred to as the Land Development Code, as amended, objections, recommendations and comments concerning the special exception, as described below, will be heard by the Board of Adjustment of the City of Starke Florida, herein after referred to as the Planning and Zoning Board, serving as the Board of Adjustment, at a public hearing on June 15, 2015 at 5:30 p.m., or as soon thereafter as the matter can be heard, in the City Hall, City Commission Meeting Room, located at 209 North Thompson Street, Starke, Florida.

LEGAL DESCRIPTION: A parcel of land lying within Section 29, Township 6 South, Range 22, being more particular described as Parcel No’s: 03956-00100, and located at 521 North Orange Ave, Starke, Florida.

The public hearing may be continued to one or more future dates.  Any interested party shall be advised that the date, time and place of any continuation of the public hearing shall be announced during the public hearing and that no further notice concerning the matter will be published, unless said continuation exceeds six calendar weeks from the date of the above referenced public hearing.

At the aforementioned public hearing, all interested parties may appear to be heard with respect to the special exception.

Copies of the special exception are available for public inspection at the Office of Building and Zoning located at 209 North Thompson Street, Starke, Florida, during regular business hours.

All persons are advised that if they decide to appeal any decision made at the above referenced public hearing, they will need a record of the proceedings, and that, for such purpose, they may need to ensure that a verbatim record of the proceedings is made, which record includes the testimony and evidence upon which the appeal is to be based.

  1. SCOPE and PURPOSE

The City of Hampton, Florida is seeking proposals from qualified sources to provide a comprehensive Water Tank Inspection and Washout for the City’s 64,000 gallons ground storage tank. The professional services are to perform water tank inspections and washout, The Professional Services Provider will identify specific items on and around the water storage tank facilities which need to be maintained or repaired, conduct an inspection and create a final report related to the existing water storage facility’s interior and exterior located in Hampton, FL. This inspection (and the drafting of the final report) will comply with the Department of Environmental Protection rules and regulations. All Professional Services shall be performed under the guidance of the designated representative of the City of Hampton, FL. The purpose of this RFP is to solicit proposals for Professional Services, and to set forth the terms and conditions whereby Hampton, FL may contract with the successful Service Provider

(s) (Proposer) (s) to provide the water tank inspection, and washout as described herein. The City of Hampton, FL reserves the right to negotiate, and contract with any firm suited to provide the Professional services described herein.

The water storage tank is owned and operated by Hampton, FL. The tank is 64.000 gallon ground storage tank is approximately 28 feet in height and 20 feet in diameter. This program will establish a report detailing a long term schedule of specific maintenance activities which need to

be performed in order to maintain and protect the integrity of this water storage facility. All recommendations for work and coating systems must meet or exceed current standards established by AWWA and as required by the Florida Department of Environmental Protection (FDEP) approved standards for drinking water.

The selected Service Provider shall provide all materials, labor, and services necessary to fulfill all obligations of the contract resulting from this RFP: these include, but are not limited to: water tank inspection and cleaning. This will include, but may not necessarily be limited to, yearly tank inspections, preparation of maintenance specifications, plans, schedules, studies, cost estimates, and reports to be submitted to the Hampton, FL Chairman of the City Council.

NOTICE IS HEREBY GIVEN that Bradford County S.H.I.P. Program will receive sealed bids until 2:00 p.m. June 22, 2015 at the Bradford County Office of the County Clerk located at 945 North Temple Avenue, Starke, Florida 32091 for the following:
