  1. SCOPE and PURPOSE

The City of Hampton, Florida is seeking proposals from qualified sources to provide a comprehensive Water Tank Inspection and Washout for the City’s 64,000 gallons ground storage tank. The professional services are to perform water tank inspections and washout, The Professional Services Provider will identify specific items on and around the water storage tank facilities which need to be maintained or repaired, conduct an inspection and create a final report related to the existing water storage facility’s interior and exterior located in Hampton, FL. This inspection (and the drafting of the final report) will comply with the Department of Environmental Protection rules and regulations. All Professional Services shall be performed under the guidance of the designated representative of the City of Hampton, FL. The purpose of this RFP is to solicit proposals for Professional Services, and to set forth the terms and conditions whereby Hampton, FL may contract with the successful Service Provider

(s) (Proposer) (s) to provide the water tank inspection, and washout as described herein. The City of Hampton, FL reserves the right to negotiate, and contract with any firm suited to provide the Professional services described herein.

The water storage tank is owned and operated by Hampton, FL. The tank is 64.000 gallon ground storage tank is approximately 28 feet in height and 20 feet in diameter. This program will establish a report detailing a long term schedule of specific maintenance activities which need to

be performed in order to maintain and protect the integrity of this water storage facility. All recommendations for work and coating systems must meet or exceed current standards established by AWWA and as required by the Florida Department of Environmental Protection (FDEP) approved standards for drinking water.

The selected Service Provider shall provide all materials, labor, and services necessary to fulfill all obligations of the contract resulting from this RFP: these include, but are not limited to: water tank inspection and cleaning. This will include, but may not necessarily be limited to, yearly tank inspections, preparation of maintenance specifications, plans, schedules, studies, cost estimates, and reports to be submitted to the Hampton, FL Chairman of the City Council.

  1. This procurement shall be conducted in accordance with the competitive negotiation policy and procedures set forth below.

NOTICE OF SECOND PUBLIC HEARING AND STATEMENT OF PROPOSED COMMUNITY DEVELOPMENT BLOCK GRANT COMMERCIAL REVITALIZATION

PROJECT

Bradford County is applying to the Florida Department of Economic Opportunity for a grant under the Commercial Revitalization category in the amount of $750,000 under the Small Cities Community Development Block Grant Program. At least 70% of the funds must be for activities that benefit low- and moderate-income persons.

The cost and percentage of low- and moderate-income persons benefitting from each project activity is as follows. The cost for the Construction activity for Housing Rehabilitation/Demolition/Replacement is $632,500 with 100.0% low and moderate-income persons benefitting from this activity. The cost for the Temporary Relocation is $5,000 with 100.0% low and moderate income persons benefiting from this activity. The cost for the Administration activity is $112,500. The total Community Development Block Grant cost for this project is $750,000 with at least 100.0% low- and moderate-income persons benefitting from this project.

The plan to minimize displacement of persons as a result of planned Community Development Block Grant funded activities is to develop a Community Development Block Grant project which will not cause displacement of persons. The proposed Fiscal Year 2014 Community Development Block Grant project will not permanently displace any persons.

If for any reason persons are permanently displaced as a result of Community Development Block Grant funded activities, assistance will be provided to displaced persons as required under 24 Code of Federal Regulations 42, United States Department of Housing and Urban Development regulations that implement the Uniform Relocation Assistance and Real Property Acquisition Policies Act of 1970.

A Statement of the proposed Fiscal Year 2014 Community Development Block Grant project application is as follows:

The Community Development Block Grant project is a housing program. The County will undertake housing improvements throughout the unincorporated area of the County. The housing improvements will include housing rehabilitation/demolition and replacement.
